اللَّهُمَّ عَافِنِي فِي بَدَنِي، اللَّهُمَّ عَافِنِي فِي سَمْعِي، اللَّهُمَّ عَافِنِي فِي بَصَرِي، اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْبَرَصِ وَالْجُنُونِ وَالْجُذَامِ وَمِنْ سَيِّئِ الْأَسْقَامِ

Transliteration:

"Allahumma 'afini fi badani, Allahumma 'afini fi sam'ee, Allahumma 'afini fi basari, Allahumma inni a'udhu bika min al-barasi wal-jununi wal-judhaami wa min sayyi'il asqaam."

Translation:

"O Allah, grant me health in my body. O Allah, grant me health in my hearing. O Allah, grant me health in my sight. O Allah, I seek refuge in You from leprosy, insanity, deformity, and from all evil diseases."
